Understanding Our Billing and Payment Procedures

At The Care Team Primary Care, we are committed to providing high-quality healthcare services while ensuring transparency and clarity in our financial policies. By signing below, you acknowledge and agree to the following:

  1. Insurance Coverage: We participate in a variety of insurance plans. It is your responsibility to provide accurate and up-to-date insurance information, including policy numbers and contact information for your insurance company. Please be aware that coverage may vary, and certain services may not be covered by your insurance plan.
  2. Co-Payments and Deductibles: Co-payments, deductibles, and any other applicable fees are due at the time of service. We accept various forms of payment, including cash, checks, and credit/debit cards. Failure to pay co-payments or outstanding balances may result in additional fees or collection actions.
  3. Out-of-Network Services: If you receive services that are not covered by your insurance plan or if you are uninsured, you will be responsible for the full cost of services rendered. We offer financial assistance options and payment plans for patients facing financial hardship. Please contact our billing department for more information.
  4. Billing Statements: You will receive a statement of services rendered and any outstanding balances after each visit. It is your responsibility to review these statements for accuracy and promptly address any discrepancies or concerns with our billing department.
  5. Payment Responsibility: As the patient, you are ultimately responsible for all charges incurred for services rendered, regardless of insurance coverage. This includes any charges deemed not medically necessary by your insurance company or services denied for coverage.
  6. Insurance Claims: We will submit claims to your insurance company on your behalf. However, we cannot guarantee payment or coverage of services by your insurance company. It is your responsibility to follow up with your insurance company regarding the status of claims and any outstanding balances.
